How to request a copy of your health records

Clients have the right to request access to their health care records.To contact us about requesting  your records:

1. Submit a written request by email or through the secure client portal.

2. Include your full name, date of birth, and the specific records you are requesting.

3. Requests may be subject to identity verification and applicable fees as permitted by law.

 

Records will be provided within the timeframe required by Texas law.If you have questions about requesting records, please contact the practice directly using the contact information listed on this website.

How to contact the licensing board

To speak with the licensing board or to file a compliant against a therapist, you may do so through the Texas Behavioral Health Council. You may reach them by US Post at George H.W. Bush State Office Bldg., 1801 Congress Ave., Ste. 7.300, Austin, TX 78701 or by phone at (512) 305-7700
