The contact form native to Squarespace can be used for initial contact, but please note that it is not HIPAA-compliant and should not be used to enter personal health information. Please use the forms on my contact page for HIPAA-secure privacy.

Client forms embedded on my contact page HIPAA-compliant and can be used to communicate personal/protected health information. These forms include contact forms, feedback forms, financial and legal agreement forms, and other forms necessary for conducting the business of this company.
